NWLSD and Hamilton County Public Health are partnering to host
FREE Covid-19 Mobile Vaccination Clinics
on Tuesday, September 7, 2021 and Friday, September 10, 2021
Who Can Attend? : This event is open to the public. While this event was designed with students and staff in mind, interested community members can also attend and be vaccinated. This is not a required event for any staff or students. Participation is 100% voluntary. Students who wish to participate, must have parental consent.
What is being offered? : Covid-19 vaccinations. Hamilton County Public Health professionals will be on-site to answer your vaccine related questions, provide educational materials and give vaccines to those eligible, interested and consented (if a minor) on a voluntary basis. Vaccines will not be administered by NWLSD Staff.

INDIVIDUALS UNDER AGE 18:
Do parents need to be present during vaccine administration? : No. However, if a parent would like to be present, they can call the school building to inform them of your planned arrival time. Your child will be pulled from class and allowed to go to the van to be vaccinated with the expectation that they return to class after their 15-minute monitoring period has ended.
What forms do I need to fill out for my minor child? : If you would like to sign your minor child (aged 12 and up) to be vaccinated, here are the steps you need to follow:
- Parents should read the Vaccine Information Sheet, this form is provided by the manufacturer and contains important information about the vaccine:
2. Parents must complete the vaccine consent and be sure to sign the form and send it to school with your child on vaccination day. No vaccines will be given without completed consent forms.
What does it cost? : This event is FREE to everyone, no insurance required.
NO APPOINTMENTS NEEDED:
Individuals should simply show up during the clinic hours to receive a vaccine. There is a 15 minute monitoring period in the van area, immediately following the vaccine being given, to monitor for adverse reactions. Once this is completed, those who get vaccinated will be allowed to leave the area.
